potentials and twitch force were measured in muscle strips obtained from patients and controls. Of the 36 patients, 25 presented with chronic muscle weakness of varying degrees, up to wheelchair-dependence. The weakness was associated with intracellular Na overload and edema. Older patients revealed a vacuolar myopathy or a progressive muscular dystrophy. Weakness, intracellular Na overload and edema were increased and further raised by cooling and glucose/insulin, and almost completely normalized by 4 weeks of treatment with the carbonic anhydrase inhibitor acetazolamide (Jurkat-Rott et al., 2009). In vitro, the chronic weakness correlated to membrane depolarization, and acetazolamide repolarized the membrane and restored force. We conclude that membrane depolarization associated with intracellular Na overload and edema causes both episodic and permanent muscle weakness. The chronic weakness is reversible in muscles which show mild or only moderate fatty degeneration. Acetazolamide has direct and beneficial effects on weak muscle and can markedly improve both forms of weakness. In addition, we tested whether the edema in Duchenne muscular dystrophy (DMD) is caused by an osmotic effect due to increased myoplasmic Na content or by inflammation. The muscle edema was quantified on STIR images using background noise as reference. Na was quantified by a muscular tissue sodium concentration (TSC) sequence. A novel inversionrecovery (IR) sequence allowed us to determine mainly the myoplasmic Na by suppression of the extracellular 23Na signal, e.g. from vasogenic edema. Both intracellular TSC and water content were markedly increased in DMD compared to volunteers (p < 0.001). We conclude that the elevated myoplasmic Na concentration in DMD is osmotically relevant and causes a mainly intracellular muscle edema that contributes to the pathogenesis of DMD. We hypothesize that antiedematous treatment can reverse the edema and prevent the edema-induced muscle degeneration.

بررسی جهش در اگزون 8 ژن CLCN1 در بیماران ایرانی مبتلا به میوتونی غیر دیستروفیک

Background: Non-dystrophy myotonias (NDMs) have similar clinical signs of muscle weakness and congenital myotoniais typical example. This disease is caused by mutations in CLCN1 gene. CLCN1 gene has 23 exons and exon 8 is hotspot. Mutations in skeletal muscle chloride channel gene are associated with a group of clinically overlapping diseases by alterations in the excitability of the sarcolemma...

Myotonia congenita and myotonic dystrophy in the same family: coexistence of a CLCN1 mutation and expansion in the CNBP (ZNF9) gene.

Myotonia is characterized by hyperexcitability of the muscle cell membrane. Myotonic disorders are divided into two main categories: non-dystrophic and dystrophic myotonias. The non-dystrophic myotonias involve solely the muscle system, whereas the dystrophic myotonias are characterized by multisystem involvement and additional muscle weakness. Each category is further subdivided into different...

Mutation analysis in exons 22 and 24 of SCN4A gene in Iranian patients with non-dystrophic myotonia.

BACKGROUND Non-dystrophic myotonias are a heterogeneous set of skeletal, muscular channelopathies, which have been associated with point mutations within sodium channel α-subunit (SCN4A) gene. Because exons 22 and 24 of SCN4A gene are recognized as hot spots for this disease, the purpose of the study is to identify mutation in exons 22 and 24 of SCN4A gene in Iranian non-dystrophic myotonias pa...

Muscle MRI reveals distinct abnormalities in genetically proven non-dystrophic myotonias☆

We assessed the presence, frequency and pattern of MRI abnormalities in non-dystrophic myotonia patients. We reviewed T1-weighted and STIR (short-tau-inversion-recovery) 3T MRI sequences of lower limb muscles at thigh and calf level in 21 patients with genetically confirmed non-dystrophic myotonia: 11 with CLCN1 mutations and 10 with SCN4A mutations, and 19 healthy volunteers. The MRI examinati...

Non-dystrophic myotonias and periodic paralyses. A European Neuromuscular Center Workshop held 4-6 October 1992, Ulm, Germany.

Our understanding of the pathology of the nondystrophic myotonias and the periodic paralyses has profited immensely from the use of modern electrophysiology (three microelectrode voltage clamp, patch-clamp techniques) and molecular biology (candidate gene approaches in contrast to reverse genetics in other neuromuscular diseases). In the past few years it has become clear that--apart from the n...
